Building is falling apart, not my first time here but alot of new faces. Attempted to talk to the director of nurses and the administrator but they were to busy for me to address concerns. The 1 star is for Karla who helped me when I felt hopeless and helped me with getting my families information after losing my phone. Also the admissions girls made me feel at ease coming here and we're great but won't be recommending in the future. So glad to be out of there.
My mom was at the facility for less than 48 hrs. Her meds were mismanaged 4 times I drive to my mom at 2 am in the morning. She had been waiting 2 hrs for her meds. The staff acknowledged that my mom was to get her meds every 4 hrs. I was reassured at 2 am my mom would get her meds at 6 am. She didn't get the meds till 9 am after I showed up cause my mom called me crying in pain. They didn't have ice packs which is weird and so gave my mom ice in like a baggie and a pillow case that leaked everywhere My mom was a fall risk and when I showed up there was still water on the floor. She didn't get the meds till 9 am after I showed up cause my mom called me crying in pain. They didn't have ice packs which is weird and so gave my mom ice in like a baggie and a pillow case that leaked everywhere My mom was a fall risk and when I showed up there was still water on the floor. Despite knowing my mom didn't have use of her left leg no staff helped EMS to transfer my mom from her bed to the stretcher and my mom fell only then did a staff member step in My mom would ask for her meds in advance to give them time cause she understands they are busy but she'd still be neglected for hours When my mom called for assistance staff would let my mom's roommate distract them for long periods just for chitter chatter. Some would even say "I need to help her" but then would still talk to the roommate instead of the patient requesting help. The room my mom was placed in had cockroaches and one even crawled over her. We asked for a different bed with sturdier rails for her safety and comfort and she was never given one in the time she was there. My mom needs to go to another Rehab center and it won't be this one. My mom is a high risk individual and doesn't deserve this type of treatment.
The 1 stars are for the staff who are kind but understaffed. The problem is with rehabilitation. My mother spent a month there and came out no better than when she came in. She can't even stand up. This is simply a warehouse for sick people. They now won't return calls. Don't trust them.
I'm really enjoying my stay here, while my broken hip heals up. The staff are all very kind and have been so so helpful in my recovery. I've got to give a shout out to Debbie, who makes me smile every time she has my room in her rounds. The food is pretty good, especially breakfast(I wish I could request it for every meal). I could tell it was going to be a good place to be staying while I heal up, when I saw how happy and high spirited most of the patients were on my first day. All in all it's a really great place. I wish there were more smoke breaks, but it's actually a good thing that there aren't because I'm smoking a whole lot less. A big thanks to everyone here at haven for making my stay so comfortable, and for putting up with me when I've been in a foul mood.
If I could give 0 I would. There's homeless camps everywhere and today 1 of them was actually in the facility. They stopped him on his way out but he shouldn't have been there in the 1st place. It smells disgusting. People are always going in to wrong rooms and disturbing patients. The physical therapy sucks. The pt is always yelling at my mom about her breathing when she's on OXYGEN! The power went on on Saturday night, so her oxygen concentrator stopped. The call lights don't work during a power outage. She tried yelling but without o2 she can't be too loud. The nurse working didn't come check on the patients at all. My mom was without her o2 for 3 hours. She could've died. They told my mom the nurse that night didn't know to come check on the patients. Excuse me? That is just common sense! This place is awful and I want them to transfer her somewhere else. She's just getting worse in this place.
We love Haven. Ted is a genuine man with a huge caring heart. Thank you Ted for always making sure the employees/patients are well taken care of. Thank you for all you do.
Horrible and dangerous facility!! My mother with copd was accepted here with the understanding they could provide for her needs. The first three days her required medication was not provided, as well as the liters of oxygen she needs. We argued with staff for those three days until she got a portable oxygen tank and her medication shes needed to take for years. She was left to suffer. We asked to go to another facility and they neglected to make arrangements. We asked to go home dr stanke said he needed documentation of her struggles to breath, additional reasons we couldnt leave. Needing rehab, physical therapy(none provided for the first six days), finally stating me (caregiver) needed training by them or they could not releases my mother to me. Called outside agencies to force a release. No cpap, no adequate nebulizer, oxygen concentrators only go to 5 liters, horrible processed meat for every meal other food is ok.
My 90-year-old father suffered several strokes and contracted influenza. Doctors gave him 1-2 days to live. I moved him to Haven Health Phoenix for Hospice Care until his passing. I found the staff there to be professional and SO kind and caring to both my father and me. It was a difficult time and they went out of their way to reassure me. The staff responded to my questions and requests quickly and efficiently. He was well cared for and treated with the utmost respect at all times. It was such a comfort to know how well my father was treated in his final days.
