I was just there at Lake Pleasant Post Acute and Rehabilitation for six weeks, and I will say , I loved it there. The whole staff was amazing. They helped me get better. I personally don’t like to stay in facilities like these but they made it comfortable for me to stay until my treatments were complete. If you’re looking for a place that only has one nurse to a patient then keep them at home and give them that care. The nurses have more than just your family member to tend to so they stay busy. I personally don’t like to stay in facilities like these but they made it comfortable for me to stay until my treatments were complete. If you’re looking for a place that only has one nurse to a patient then keep them at home and give them that care. The nurses have more than just your family member to tend to so they stay busy. They are not lazy. I would definitely recommend this facility, but honestly I would keep my loved ones home and take care of them myself instead of pawning them off to a facility and then complaining about it because of ignorance.

My Husband was here in December 2023. His bed was shoved against a wall facing a tv that worked less than 25% of the time. He was there for PT/OT which he did for 30-45 minutes a day 5 days a week. The rest of the time he was in his bed, doing nothing. Many of the staff just completely ignored his call light and would not assist with meals or toileting. He lost 15 pounds...due to lack of assistance with meals and no one botherd to find out what he would eat. Rooms are clean which is nice. Food is adequate but always cold I don't understand what goes on with the workers here. Is it laziness? When they are available the "I don't want to be here shows." Some residents need to be fed. The food gets put aside to sit somewhere and get cold. Then the cold food is collected without the resident getting fed. This is a difficult job no doubt. If you don't want to work and help these people; then don't work there.

This is a horrible place. my husband spent about 7 days there. Fell on his head and shoulder twice. Was left on mattress on floor lying down with foodtray on his stomach. No one to help him. Fell twice and team there were not allowed to chart it. But some told me about it and his roommate told me also. I had them send himto the hospital. He could not think clearly. Do not send love ones who need wheelchair or can't walk well, You will be so sorry. This shld have minus stars but there are none

My sister is blind and was at that facility for a approximately 8 days or so . They were very rude and uncaring. They would bring her food in to her and not even tell her it was there or where it was on the plate. She was unable to get up out of bed because she had surgery on her leg. So she was totally dependent on there help . She would push her help button and no response for 2 to 5 hours or not at all. The night CNA's would scream at her when she would push her call button because she a bowel movement. Again this would take them hours to respond then they would belittle her .I would never ever take anyone I loved to this place . It's a horrible facility and very unprofessional. I will calling the blind association of Arizona to fill a grievance against them. I wish I had checked reviews before I agreed to let her go there . I personally removed her on a Saturday and asked for her info for a doctor appointment for Monday and they refused give them to me . STAY AWAY !!!

My sister was a resident there for approximately nine days, give or take a few. I would not take a dog that I didn't like there. My sisters blind. They never told her when her food was there. She would sit and wait for hours for her food to be served, while it would be sitting right next to her, obviously unbeknownst to her. They never told her where the food was the on the tray. They let my sister lay in her poop in dirty diapers for hours after she paged them. The night CNA's were absolutely obnoxious they would scream at her. The people during the day were not too bad, but overall that place is a piece of crap. I have no idea how they stay licensed the way they treat people, especially people that have disabilities. If I had a family member that needed care, that would be the last place I would ever take them. My sister and I plan on filing a grievance with the blind association with the state of Arizona in reference to this facility. It's sad that they stay in business and for what they charge it's a pitiful facility. I asked for her records for a doctors appointment that she had on Monday. I had picked her up on Saturday . They refused to give them to me. Again I would never take anybody that I love there to be taken care of. It's an absolute disaster.

My Brother has been at this place going on 2 weeks. He is supposed to be getting speech therapy, physical therapy and occupational thereapy. He has had 1 speech therapy session which is one of the biggest issues. Other then that it has been one therapy session a day for barely 30 minutes and no weekends at all. The food in there i wouldnt feed to my dogs and not enough for a child. Only allowed 2 showers a week!!! The rooms are only the size for a single person both patients are against walls. Hopefully neither need emergency help because they would be twice as long rendering aide. This place is unsafe and very unheathly. Please do not take anyone you love to this place. Its not really the people who work there its the management that allows this to happen, totally disgusting!!!!!

Bad at nursing and basic medical care. Communication between staff is horrible. Mother entered facility for rehabilitation and passed away 11 days later. She had a Broken hip and arm. Many visits she had no dentures in or eye glasses on. Facility administrator did not respond to concerns.
