During her time there, PT would come in, get her into a chair, and leave her for an hour, if not more, at a time alone. They did not do exercises with her, and as a max assist patient, left her to complete them herself. The cafeteria workers would also come and leave her dinner on a table, out of arms reach, and come back 10 mins later and take it away. My mom had to attend their SNF program due to falling at home many times. They marked my mom down as “refused to eat” when she was not given the help she needed to eat the food but was also on a soft food diet, due to swallowing issues. Because she wasn’t eating, her doctor ordered a calorie supplement drink to be given at each meal time, sometimes she wouldn’t get that for days. Other time, she would be left with no diaper on. When we went to ask why that was, the nurse said “she took it off herself” which would be impossible for her to do as a max assist patient. Lots of times when we went to the nurses with questions and issues, we were told “that’s not my problem, it’s someone elses”. My mom was depressed the whole time she was a patient so we requested the therapy dog make a visit to her room, not once did they visit or anybody come talk to my mom. Since her leaving the facility (we took her out), I have called numerous times asking to discuss the care that was given to her while she was there with the Director of nursing, and I have been told “they aren’t in at the moment” and “I don’t know when they will be available, I don’t know their schedule”, and have not received a call back. If I could give this place 0 stars, I would.
Thank you to everyone for your hard work, dedication, and the exceptional care you provide every day. I would confidently recommend this skilled nursing facility to anyone looking for quality rehabilitation or long-term care for a loved one.
They're doing a wonderful job, the team is helping me with therapy, the social worker team is helping me with personal affairs and getting me back on my feet! Everyone is super friendly, my roommate is fun and we get along!

They are a joke!!! Avoid at all cost!! They lost our family members clothes & took the snacks we provided. They changed the type of medical device he needs to fit what the facility has on hand instead of what is ordered by the DR. His AHCCS insurance coverage has changed mid treatment to cover the new medical disabilities, so the facility moved him into a back room, stopped all physical/ occupational therapy, stopped all assistance in/out of bed into his wheelchair and decreased daily personal supportive services until the finance office gets the insurance documents in the system. This is week three now and still no physical therepy /rehabilitation training. The case manager & therpy supervisor never answer or return phone calls. Patients are treated like cattle not like human beings.
I love when Cooper, our therapy dog comes and visits. I love activities, very friendly and helpful. Activities visits to our room makes me happy!! Nursing staff I love! Therapy is going very well.
1:30 am and no one is available to take my call? If my loved one is there I should be able to get a status update! She's not even there yet. I called 1. To see if anyone would answer and 2. If I could get midnight updates. Im ticked!!! Not one person answered the phone. 😡
The staff and people here are wonderful. It’s a great place to help your loved one get back to their baseline.
I had to come to Sandridge to receive PICC line antibiotics and PT, OT following major surgery on my leg. The staff here are really friendly and kind. Multiple daily activities and outdoor patios keep you from going stir-crazy. Visitors are welcome and encouraged to come.
