I was looking forward to a new journey after 2 surgeries and being in the hospital for 3 weeks. This is not the place to go. I was forgotten for 3 meals. An hour to get a drink of water. No one ever got me out of bed. People screaming for a nurse. TVs blaring all night. Nutrition for healing was nonexistent. I cried myself to sleep the first night. I had 1 staff my last night who asked if I needed anything. No air window open sweated all night
The staff are caring but there are not enough staff. Wait time for someone needing to be helped to the bathroom is minimum 45 minutes. Breakfast dishes are not taken away until lunch is brought. Food is often cold. It tastes like they microwave it. Nothing made in house.
We chose Osborn Health and Rehab for our mother because of its Medicare rating and location. The website pictures of the inside facility, and their "spacious private rooms" was not what we found after our mother arrived and unable to walk. She was in a semi-private room that couldn't accommodate a visiting family member because of its small size. It was unlit, dingy, the food was horrible, staff was limited, response times were not always good. After the first night, she awoke to find an aide going through her purse. When calling the facility, many times no one even answered the phones.…
When calling the facility, many times no one even answered the phones. Leaving messages did not result in call backs. Very little was done to get her up and walking again. We transferred her to another skilled nursing facility, but that process took many days to happen, unfortunately, resulting in her being weaker and weaker. For those looking for skilled nursing facilities, make sure you visit them before placing them anywhere. Read everything you can about the facilities including the reviews. I do not recommend this facility.
I’m sorry to say this was the worse experience I’ve ever had. I was there 24 hours and probably cried 20 of it. My room was disgusting, when the A/C kicked on it sounded like a plane landing, the 9” TV was maybe 9” from the ceiling and didn’t work, toilet handle was broken, food was not edible. Adjusting the bed sounded like it was going to break. Staff was nice, but not able to help with these issues. I left the facility because I knew this was not a place that was going to be able to assist me in my recovery. Being comfortable is part of recovery and this was far from that. Don’t let the picture of the room on website fool you!
My father was there as well. Yes Julie it smelled. There are for sure under staffed. My father was a bit of a pain . Regardless he was ignored alot. They will tell you the opposite. If you call have lots of Patients. They say there's a night staff but it's maybe one person in your lucky if you get a visit once a night. So if they have extreme health conditions this would not be the place. It's very much a routine for the nurses and a lot of them are not pleasant especially if you are. Corinne was a good nurse. But towards the end she had no level of patience because my dad was a bit flirtatious.. Briefly in which he apologized. May I add he hasn't walked in for years and I was lonely. My suggestion is you pick the job if you don't like it move on. They're human beings.
I spent 7 weeks here after having sepsis. I hadn't walked for months and my doctor recommended a rehab stay after leaving the hospital. The PT department is fantastic and helped me to walk again after a few short weeks. They made me feel so good about myself every time I passed a new milestone. I can't say enough about the wonderful PT team that helped me get back home.
Yes, I am a current patient. I was transferred from the hospital without a choice. This place is disgusting, nurses very bad about giving out medication, the day staff is completely different, they are in your room almost immediately when you turn on your call light. It's after 630 pm that the trrouble starts Yesterday I rang my call light 2 hour's and 15 mins it took for me to get pain relief that night nurse. Was Bernise. Tonight at 630pm put on my call light 45 mins a cna opened my door, I told her my potty needed dumping and if she could ask my nurse to please bring me Tylenol and a muscle relaxer a hour and fourth five minutes later she shows up with my evening medications but no Tylenol or muscle relaxer she was trying to tell me I had to wait 24 hour's to take it, paulina said you can have it in the next 2 hour's I told her that was not what the order said because I got a copy from one of the nurse's when they ran out of my blood thinners over a weekend. There have been other things extremely bad food poorly prepared, in therapy they do not disinfect or clean the bikes or ball's or other equipment there between patient's. The whole place smells like urine, I pulled my call light after 45 MINS. A CNA camein turn out the light and left never dumped it. It's now been 3 hour's and 45 mins. It's still here no one will touch it until morning. Also do your own laundry they lost more that I got back from them been in the same dirt clothes now for 3 day's. Please do not ever let a loved one come to this facility, it is poorly managed and unless you watch and are able to be your own advocate check your medications carefully don't come here at all if you have a choice!
This place looks good on the outside, but the care there is abysmal. Our brother was there for one week before he died there. On his second day there, after we visited him, he called me (I was 45 minutes away) and desperately asked me to call the nursing station because he could feel his blood sugar was tanking and the nurses weren't responding to his call button. I called, and they responded, but really? On his third day there, again, he called me after we visited. He said he had woken up from a nap in the evening to a strange man sitting in a wheelchair in his room, who was disoriented (a patient who was confused), and told him he was using the internet there. He called me because, again, he couldn't get the nurses to respond. He got so frustrated that he called 911 and the police came to sort it out! I called the both nursing stations and the first three times I called, it rang 50 times and no one picked up! On the 4th call, a resident answered the phone, who then hung up on me. On the 5th call, I asked that same resident to please call the nurse as my brother was in the midst of a crisis there. 5 minutes passed, and no nurse responded. I called the director on his cell phone to inquire about the incident in my brother's room as well as to find out why on earth the residents are using the nurse's phones. He said the nurses do that sometimes! I voiced my concern about the nurses not responding both that night and the night before, and the director offered to place him in a room near the nursing station. It never happened, in spite of the director's promising to take extremely good care of him, which included, at our request, that someone make sure he was eating his meals to keep his blood sugar stable. Also on one of those days, I noticed his ankle was swollen. The nurse said it was nothing, to just prop it up on a pillow. They should have sent him back to the hospital to make sure he wasn't having an embolism, but they both didn't address it, nor did they write this fact in his medical notes. (He ended up dying of an embolism, probably exacerbated by the lack of oxygen, which he wasn't given on his last day alive.) 5 days later was his last day alive. In spite of being so weak that he could hardly hold his head up, he was left in a wheelchair away from the call button, and his meals were untouched (a fact confirmed in the autopsy), and the nurses refused to assist getting him back in bed, they were very late in getting him his insulin, and when he was finally transferred to the bed, they realized the oxygen tank had no oxygen in it. He suffocated and had an embolism as a result of their lack of care. We have filed a complaint with the licensing agency, and have sent a letter to the director. The licensing agency responded by saying they talked with the nurses, etc. and couldn't prove any wrongdoing! We also never received a response from the director, in spite of the fact that they are required to respond to complaints such as these. Based on the lack of care we received at Osborn, I would sign a DNNR (do not nursing home me) in addition to a DNR (do not resuscitate) form, if I ever find myself in this position, based on how awful our brother's care was there. We would file a medical malpractice suit if siblings could do this in Arizona, but we can't. We aren't interested in a financial settlement anyway. But we sure would like to find a way to make it so that others don't have to suffer the way our brother did. Please beware of this place. It was horrible. Beyond horrible.
